Mentor Agreement
A legal disclaimer
1. Purpose
DVM Ascent provides structured professional mentorship for veterinarians, students, and practice leaders.
This Agreement outlines the expectations, responsibilities, and standards for participation as a Volunteer Mentor within the DVM Ascent platform.
Mentorship provided under this Agreement is professional guidance and leadership development support only. It is not therapy, legal counsel, financial advice, or medical supervision.
2. Volunteer Status
Mentor acknowledges that participation in the DVM Ascent Mentor Program is voluntary.
Mentor:

Is not an employee of DVM Ascent

Is not entitled to compensation, benefits, or reimbursement unless otherwise agreed in writing

Is responsible for maintaining all applicable professional licenses and insurance
Future paid roles may be offered at the Company’s discretion but are not guaranteed.
3. Scope of Mentorship
Mentor agrees to:
• Provide structured mentorship sessions (minimum 1–2 per month unless otherwise agreed)
• Follow the DVM Ascent 4-Step Mentorship Framework
• Maintain professional tone and conduct
• Avoid providing medical, legal, or financial directives beyond general professional guidance
• Refer mentees to appropriate professionals when issues exceed scope
Mentor shall not:
• Provide medical supervision or case liability oversight
• Offer legal advice
• Represent themselves as an employee or agent of DVM Ascent
• Use DVM Ascent branding without written approval
4. Confidentiality
Mentor agrees to maintain strict confidentiality regarding all mentee discussions, personal information, and session content.
Confidential information may only be disclosed:
• If required by law
• If there is concern of harm to self or others
• With written consent from the mentee
Mentor agrees not to publicly discuss or reference specific mentees without explicit written permission.
5. Professional Conduct
Mentor agrees to:
• Uphold integrity and professionalism
• Treat all mentees with respect
• Avoid discrimination or harassment
• Maintain appropriate communication boundaries
• Avoid dual relationships that may create conflicts of interest
DVM Ascent reserves the right to remove any mentor whose conduct is inconsistent with Company values.
6. Liability & Insurance
Mentor acknowledges:
• They are solely responsible for maintaining active veterinary licensure (if applicable)
• They are responsible for maintaining professional liability insurance
• DVM Ascent is not liable for advice or guidance given by Mentor
Mentor agrees to indemnify and hold harmless DVM Ascent, LLC from any claims arising from mentorship activities.
7. Intellectual Property
All DVM Ascent materials, frameworks, branding, and training content are the intellectual property of DVM Ascent, LLC.
Mentor may use materials solely for activities within the DVM Ascent platform.
Mentor may not reproduce, distribute, or repurpose materials for outside business use without written permission.
8. Term & Termination
This Agreement remains active until terminated by either party.
Either party may terminate participation at any time with written notice.
DVM Ascent reserves the right to remove a Mentor immediately for:
• Breach of confidentiality
• Unprofessional conduct
• Misrepresentation
• Violation of this Agreement
9. No Guarantee of Referrals
DVM Ascent does not guarantee:
• Specific number of mentee matches
• Volume of sessions
• Future paid opportunities
10. Acknowledgment
By checking the text box in the applicaitons, theMentor affirms that they:
• Have read and understand this Agreement
• Agree to uphold DVM Ascent standards
• Will complete required onboarding training
• Understand this is a volunteer leadership role
